3 <br /> Between The University of North Carolina <br /> School of Dentistry for its <br /> Department of Dental Ecology <br /> and OCHD and UNC Hospitals <br /> Page 2 of 3 <br /> 1/4 the total salary ($29,000) and fringe benefits, Composite Benefits ($88.00), FICA ($2,220.00), Hospital <br /> Insurance ($2657.00), Parking ($210.00), Interns Permit ($75.00), Total Fringe ($5,250.00) of one <br /> postgraduate year one resident beginning July 1, 1994 (quarterly totals-salary$7,250.00 fringe$1,312.50 <br /> grand total quarterly reimbursement $8,562.50). <br /> 2. The School of Dentistry will provide the services of one postgraduate year one resident to The <br /> Health Department for the purpose of rendering comprehensive dental services of 4 1/2 days/wk. <br /> 3. School of Dentistry will bill The Health Department at the close of each quarter for the <br /> salary/fringe of one postgraduate year one resident. <br /> 4. The person whose services are to be provided pursuant to the Agreement is, for all purposes, <br /> an employee of The University of North Carolina Hospitals. <br /> 5. While the University of North Carolina Hospitals resident is on rotation at the Orange County <br /> Health Department, their professional liability insurance coverage will be provided by the School of <br /> Dentistry with coverage of at least $1 million, per incident, $3 million per year, <br /> 6. This Agreement shall run for a period of one year, from the 1st day of July, 1994 to the 30th <br /> day of June, 1995, and shall be renewable thereafter. <br /> 7. This Agreement or its renewals may be terminated at any time without penalty by either party <br /> provided that written notice of such termination is furnished to the other party at least 90 days prior to <br /> termination. In the event of such termination any payment due shall be prorated to the date of <br /> termination. <br /> 8. In compliance with 42 U.S.C. 1395x (v)(1)(I) and implementing regulations, The School of <br /> Dentistry and The Health Department agree, until the expiration of four years after the services are <br /> furnished under this contract, to allow the Secretary of the Department of Health and Human Services and <br /> the Comptroller General access to this contract and to the books, documents and records of The School <br /> of Dentistry and The Health Department necessary to verify the nature and extent of this contract. <br />
