2 <br /> ORANGE COUNTY C.A.T. PROGRAM <br /> AGENCY PARTICIPATION AGREEMENT <br /> This agreement entered into this 15th day of November, <br /> 1994 by and between the County of Orange on behalf of the <br /> Department on Aging, hereinafter referred to as the County <br /> and the Piedmont HIV Healthcare Consortium, hereinafter <br /> referred to as the Piedmont Consortium. <br /> The purpose of this agreement is to outline the conditions <br /> under which the County who administers the C.A.T. <br /> (Coordinated Area Transportation) Program and the Piedmont <br /> Consortium will cooperatively provide human service <br /> transportation to Orange County's elderly, infirmed, and <br /> disadvantaged as a participating member of the C.A.T. <br /> Program. <br /> SPECIFIC RESPONSIBILITIES OF ORANGE COUNTY <br /> 1. To assign professional staff within the Department on <br /> Aging to administer and coordinate the C.A.T. Program. <br /> 2 . To monitor CAT Two-Way Radios installed in <br /> participating agency vehicles. <br /> 3. To arrange bulk gas purchases for agency CAT vehicles. <br /> 4 . To provide short term backup loan vehicles for Agency CAT <br /> vehicles temporarily out of service. <br /> SPECIFIC RESPONSIBILITIES OF Piedmont Consortium <br /> 1. To operate and schedule the use of participating C.A.T. <br /> agency vehicles(s) on behalf of human service clients and <br /> approved groups, according to the Orange County <br /> Transportation Board policy and procedures. <br /> 2 . To allow C.A.T. Program agencies access (time/ridesharing) <br /> to assigned vehicle(s) where appropriate and not scheduled <br /> for use at a rate established by the County Commissioners. <br /> 3. To complete all required vehicle operation reports and <br /> submit to the Orange County C.A.T. Supervisor in the <br /> Department on Aging. <br /> The term of this contract shall be from November 15, 1994 <br /> and will continue indefinitely, unless sooner terminated by <br /> either party giving 30 days prior written notice to the other <br /> party of intention to terminate. `` <br /> IN WITNESS WHEREOF, Orange County and Piedmont Consortium <br /> have executed the Agreement of the date first above written. <br />
