Said registered <br /> nurses shall be an employee and agent of the Home Health Agency of Chapel <br /> Hill, inc. and shall not be an agent or employee of Orange Cbunty. <br /> II. That the Home Health Agency shall maintain proper documentation <br /> and records as necessary for all visits perfammeheumunder and shall submit <br /> docurrentation to DSS within 30 days of each client contact. <br /> • III. That the Home Health Agency will provide workers' compensation <br /> insurance on all employees assigned to this program and shall insure that <br /> adequate liability insurance is carried on all motor vehicles used in <br /> connection with this program for the limits on the liability, $1,000,000 - <br /> $1,000,000. <br /> IV. That the Department of Social Services shall pay unto the Home <br /> Health Agency of Chapel Hill, Inc. the sum of $17.00 per hour for nurse's <br /> consultation and home visits, included in this cost is a travel allowance of <br /> 200 miles per month. The Hare Health Agency shall submit to the DSS on or <br /> before the fifth day of each month a bill for the previous month's <br /> activities. <br /> V. This contract Shall be effective and shall continue until terminated. <br /> It may be terminated by either party by giving the other notice thirty (30) <br /> days advance notice of its intention to terminate. <br /> • <br />
