MEET, made and entered into this 15th day of June 1988, by and <br /> between GRANGE COUNTY (hereinafter referred to as the (OUNTY) and the HOME <br /> HEALTH XENCY OF CHAPEL HILL, INC., Orange County, North Carolina. <br /> WITNESSETH: <br /> WHEREAS, the parties have agreed with each other that the Home Health <br /> Agency of Chapel Hill, Inc. will provide certain services for the Orange <br /> County Department of Social Services (hereinafter referred to as DSS) in <br /> connection with the In-Home Servi6e Program; and <br /> igHEREAS, the COUNTY has agreed to pay certain compensation for said <br /> service and the partiesdesire to execute this contract to delineate their <br /> understanding of their agreement; <br /> NOW, THEREFURE„ IEE PARTIES HEREBY AGREE AS FOLLOWS: <br /> I. That the Hans Health Agency will at the request of the Department of <br /> Social Services, provide a registered nurse to (A) assess needs of clients in <br /> medical and psychological areas; (B) provide consultation on specific cases, <br /> (1) in client's home or facility, (2) in-office, (3) informally answer <br /> medically related questions and (4) consultation an rest bane client issues; <br /> (C) shall assist with short term Adult Protective Services decisions regarding <br /> health and mental health risks; 00) participate in client case reviews for <br /> service appropriateness; (E) prepare DSS staff for inter-agency team duties <br /> involving health related issues and questions, and serve as a team umber as <br /> requested by DSS; (F) shall be amember of intra-agency team(s); (C) shall <br /> read/interpret ELF-2's with social workers—suggest when other levels of care <br /> might be appropriate or possible; (H) serve as liaison with physicians; <br /> (I) shall be an educator for chore staff and rest home operators and (J) shall <br /> • <br />
