40 <br /> FTA FISCAL YEAR 2016 CERTIFICATIONS AND ASSURANCES <br /> CATEGORY 15. SENIORS AND INDIVIDUALS WITH DISABILITIES <br /> PROGRAMS. <br /> Before FTA may provide federal assistance for an Award financed under the Formula <br /> Grants for the Enhanced Mobility of Seniors and Individuals with Disabilities Program <br /> authorized under 49 U.S.C. §5310, as amended by the FAST Act,you must select the <br /> Certifications in Category 15 in addition to other Certifications and Assurances you must <br /> select on your Applicant's behalf, except as FTA determines otherwise in writing. <br /> Any provision of the Certifications in Category 15 that does not apply will not be <br /> enforced. <br /> 1. The following Certifications for the Formula Grants for the Enhanced Mobility of <br /> Seniors and Individuals with Disabilities Program are required by 49 U.S.C. § 5310. <br /> Therefore, except as FTA determines otherwise in writing, on behalf of your <br /> Applicant, you certify that: <br /> a. Each Subrecipient is: <br /> (1) A private nonprofit organization, or <br /> (2) A state or local governmental authority that: <br /> (a) Is approved by a state to coordinate services for seniors and individuals <br /> with disabilities, or <br /> (b) Certifies that there are no private nonprofit organizations readily <br /> available in the area to provide the services authorized for support under <br /> the Formula Grants for the Enhanced Mobility of Seniors and <br /> Individuals with Disabilities Program. <br /> b. It will comply with the following selection and planning requirements: <br /> (1) The Projects it has selected or will select for an Award or Subaward of <br /> federal assistance appropriated or made available under 49 U.S.C. § 5310 are <br /> included in a public transit-human services transportation plan that has been: <br /> (a) Locally developed, and <br /> (b) Coordinated. <br /> (2) The public transit-human services transportation plan was developed and <br /> approved through a process that included participation by: <br /> (a) Seniors, <br /> (b) Individuals with disabilities, <br /> (c) Representatives of public,private, and nonprofit transportation <br /> providers, <br /> (d) Representatives of public,private, and nonprofit human services <br /> providers, and <br /> (e) Other members of the public. <br /> (3) Within its Award, the Projects selected to receive federal assistance will <br /> assist in providing transportation services for seniors and individuals with <br /> 35 <br />
