2. Additional personnel, if any, who will be required for full-time <br /> employment or on a sub-contract basis must be identified. The <br /> technical areas, character and extent of subcontract or <br /> consultant activity will be indicated. <br /> Cost Proposal <br /> The Proposer must submit a Cost Proposal fully supported by cost and pricing data <br /> adequate to establish the reasonableness of the proposed fee. TRSWPC will require <br /> a three year Contract, which is contingent upon: (1) participating local governments <br /> authorizing funds each year; (2) the successful annual performance of the contractor; <br /> and (3) an agreement to maintain prices for the first year of the Contract period. <br /> Costs should be addressed as separate categories on a per facility basis. The <br /> Proposer shall he required to demonstrate in the Cost Proposal costs for each County <br /> by oinll entering into a contract with a single contractor and, on-the other hand, <br /> costs for each County by contracting separately. Cost should be delineated as <br /> follows: <br /> 1. Labor <br /> A. On site labor to service the collection facility. Specify number of <br /> personnel required arid an hourly rate for each. <br /> Cost on a task rate for initial technical assistance as required in 11 A. <br /> C. Cost on a task rate for all required and recommended training of all <br /> local government personnel during the course of the contract. <br /> D. Cost on a task rate for an on-site supervisor. <br /> 2. Labor (alternate proposal) - Complete "turnkey" staffing to include site labor <br /> and supervision on a task rate. <br /> 3. Transportation - All costs associated with transportation of Contractor to and <br /> from facilities. <br /> 4. Materials/Equipment/Supplies - Identify what would be needed for this <br /> project and the itemized expense including drums, vermiculite, etc, <br /> 5.
