The Contractor shall <br /> complete all manifest and shipping papers and shall provide such completed <br /> records within five (5) working days to each local government once receipt at <br /> a disposal facility is complete. The Contractor shall provide documentation <br /> attesting that the waste has been properly disposed of. Individual manifests <br /> must be submitted to each local government based on its respective <br /> collections. <br /> III. PROPOSAL INSTRUCTIONS <br /> Technical Proposal <br /> Each Proposer is to make a written proposal which presents an understanding of the <br /> Tasks to be Performed. The Proposer is asked to address each evaluation criterion <br /> and to be specific in presenting its qualifications. The Proposer should provide all <br /> of the information which it considers pertinent to its qualifications for this project. <br /> A. Statement of Understanding and Technical Approach to include: <br /> 1. Proposed preliminary work plan with an explanation of the proposed <br /> program for executing each of the listed Tasks to be performed. <br /> 2. Safety considerations and precautions, and spill contingency plans. <br /> 3. An opinion on the potential of shared liability among local governments <br /> related to bulking waste materials from multiple jurisdictions. <br /> In addition the Proposer may comment on any aspect of the Request for <br /> Proposal, including suggestions on possible alternative approaches to the <br /> coverage, definition, development and organization of the issues presented in <br /> the Tasks to be Performed section of this Request for Proposal. <br /> B. Statement of Personnel Qualifications <br /> Pull-time and part-time staff, proposed contractors and subcontractors who <br /> will be assigned direct work on this project should be identified. Special <br /> mention shall he made of direct technical supervisors and key technical <br /> personnel, and approximate percentage of the total time each will be available <br /> for this project. The Proposer must indicate the nature and extent of <br /> participation by any subcontractor or contractor. <br /> 1. Resumes of staff and proposed consultants are required which <br /> will indicate education, background and recent relevant <br /> experience with the subject matter of the project.
