ORANGE COUNTY C.A.T. PROGRAM <br /> VEHICLE LEASE AGREEMENT <br /> This agreement entered into this 1st day of July , 1994 <br /> by and between the County of Orange <br /> hereinafter referred to as The County <br /> and Charles House Association <br /> hereinafter referred to as Charles House <br /> The purpose of this agreement is to outline the <br /> conditions under which the County <br /> and Charles House will cooperatively provide <br /> human service transportation to Orange County's elderly, <br /> infirmed, and disadvantaged as a participating member of the <br /> C.A.T. (Coordinated Agency Transportation) system. <br /> SPECIFIC RESPONSIBILITIES OF Orange County <br /> 1. To lease the following identified vehicles or similar <br /> size at the hours specified: <br /> 1983 Ford Van with lift (1FTJS35GXDHB87342) <br /> 2 . To provide all insurance, fuel, maintenance repairs and <br /> replacement for each leased vehicle. <br /> SPECIFIC RESPONSIBILITIES OF Charles House <br /> 1. To reimburse at the per mile rate of $ .35 for a van or <br /> $.45 per mile for a minibus and $.65 for a passenger <br /> bus, due thirty days after billing. <br /> 2 . To operate and schedule the use of the leased <br /> vehicle(s) on behalf of human service clients and <br /> approved groups, according to the Orange County <br /> Transportation Board policy and procedures. <br /> 3 \ To ar all \ ease vehicle at t e CA pa ing of ( 00 <br /> Wes , Tron St , Hi lsborough whe not in e. <br /> 4 . To complete all required vehicle operation reports and <br /> submit to the Orange County C.A.T.
