5 <br /> ORANGE COUNTY C.A.T.AGENCY <br /> TAXI PROVIDER AGREEMENT <br /> This agreement entered into this first 1st day of <br /> July, 1994 by and between the County of Orange on behalf <br /> of the Coordinated Area Transportation Program (C.A.T. ) , <br /> hereinafter referred to as the County and <br /> , hereinafter referred to as <br /> SPECIFIC RESPONSIBILITIES OF <br /> 1. To provide Demand Response Transportation to approved <br /> participants at a rate not to exceed $1.35 per mile as <br /> agreed upon by the Transportation provider and the Orange <br /> County Department on Aging hereinafter known as the Lead <br /> Administrative Agency. <br /> 2. To meet all service provision standards for the <br /> Department on Aging's C.A.T. program such as required by <br /> the State of North Carolina and provide documentation <br /> upon request. <br /> 3. To ensure that such records as necessary are kept to <br /> fully disclose the extent of the service ( Mileage, <br /> Time, Client, Distance) provided to participants and <br /> available for inspection. <br /> 4 . To submit a monthly bill for services indicating <br /> distance, and cost by the client rendered payable within <br /> 30 days from date of receipt. <br /> 5. To adhere to the Department on Aging's C.A.T. Procedures <br /> in the provision of the service. <br /> 6. To indemnify and save harmless Orange County, its agents <br /> and employees from and against any and all loss, cost, <br /> damages, expense and liability caused by the failure of <br /> the Provider to fully perform its obligations under this <br /> agreement and in accordance with its terms; or by <br /> accident or other occurrence causing bodily injury, <br /> including death, sickness, products or services rendered <br /> under this Agreement. <br />
