5 . To provide the Transportation Supervisor written <br /> notification of any transportation problems or <br /> unsatisfactory driver's performance in order to take <br /> corrective action. <br /> 6 . To provide insurance coverage (liability/collision) . <br /> Proof of insurance and coverage limits shall be submitted <br /> to the Aging Director within 10 days of contract approval. <br /> 7 .
