2 <br /> Since the Office Assistant and outreach Social Worker were hired <br /> December 20, 1993, the following outcomes have been accomplished: <br /> 1. Nearly 40 contacts have been made with other providers. <br /> 2 . Both workers have attended training programs and received <br /> on-site training from State consultants. <br /> 3. Contacts and linkages have been made with other agencies <br /> such as Head Start, Women's Infants and Children Nutrition <br /> Program, Orange Chatham Comprehensive Health Service, <br /> Carolina Access and the DSS Medicaid staff, and the <br /> Community-Based Public Health Initiative. <br /> 4 . Patient records have been established on 77 EPSDT cases. <br /> 5. 90 patient reminder letters have been sent. <br /> 6. 147 patient appointment verifications have been done. <br /> 7 . 149 patient related contacts have occurred. <br /> In addition to these outcomes, the Health Check (EPSDT) program in <br /> Orange County is distinguished by its collaboration with other local <br /> programs for information sharing and coordination of services with <br /> other providers and programs. For instance, the Carolina Access <br /> Program in the Department of Social Services provides updated lists of <br /> providers who have agreed to participate in Carolina Access and the <br /> coordinators of both programs routinely discuss information about <br /> Medicaid recipients and providers. Furthermore, the Health Check <br /> Coordinators share information and tracking reports to update patients <br /> status. The Immunization Action Plan and Health Check data are linked <br /> both at the State and local level. The Office Assistant who does the <br /> tracking coordination has actually entered some of the data to complete <br /> documentation of the immunization status on Health Check patients. <br /> Thus its is clear that the Health Check program has enhanced the <br /> coordination of services for children in Orange County. <br /> On June 6, 1994, the permanent, time-limited Office Assistant and <br /> Social Worker positions were continued through September 30, 1994, <br /> using a $10,000 "bridge grant" from DEHNR. The attached memorandum <br /> outlines how the Division of Medical Assistance and DEHNR plan to <br /> provide reimbursements to health departments for Health Check <br /> Coordinator Services. Because Orange County has two coordinator <br /> positions, we will receive $5,845 dollars per month to cover the cost <br /> of the project. The specific budgeting for the continuation of the <br /> project will be based on guidance we are awaiting from DEHNR. However, <br /> it appears that the funding will be sufficient to cover the cost on the <br /> two positions on an ongoing basis. <br />
