Title III <br /> EXHIBIT B-D <br /> F <br /> IMPLEMENTATION PLAN: HOMEMAKER-HOI1E HEALTH AIDE <br /> Indicate Service) -..� <br /> • This Plan should set forth the method(s) and location(s) of service delivery, the <br /> extent of publicity for Use service, the extent and nature of coordinated relation <br /> ships with community agencies involved with similar service efforts. The Plan shoo <br /> also indicate the types of personnel involved in the service delivery, numbers of <br /> hours required and the resources (Title ill or non-Title III funds) to be used to <br /> support the persons. The method of handling any Income contributions to the sery <br /> should also be outlined. <br /> Area Covered: Orange County <br /> Method of Delivery: <br /> Orange Count Department on A• in , will use Title III funding to suppor <br /> a part-time Homemaker Coordinator (32 hours ) who will be responsible f <br /> the in-service education of homemakers , coordination for appropriate <br /> placement of participants with homemakers , and the counseling of parti <br /> cipants needing home care. The coordinator is also to be employed wit <br /> local funds for 8 hours (full position) to handle non-elderly cases , t <br /> being able to serve any request for service. The homemakers will come <br /> from a corps of personnel trained to render supportive aid to older pe <br /> sons who have lost some ability necessary to maintain independent livi <br /> These homemakers will be under the supervision of a physician, registe <br /> nurse or family member. The program will supplement the professional <br /> 40 nursing and physical therapy services provided by the Home Health Agen <br /> and the County Health Department. Older clients will be given the opt <br /> of negotiating payment for service directly with the aide or paying thi <br /> Department on Aging for one of its hired aides . <br /> In cooperation with the County ' s health agency, a brochure entitled <br /> In Home Services will be updated and distributed . Joint referrals wil <br /> exist between the Department on Aging, the Department of Social Servict <br /> the Home Health Agency , Orange-Chatham Comprehensive Health Services , <br /> and the County Health Department. Plans are underway for the Departmer <br /> on Aging Homemaker-Home Health Aides to handle all of the Home Health <br /> Agency ' s health aide service covered under Medicare , Medicaid or prival <br /> insurance. <br /> Pro.ect Income: <br /> Service participants will be allowed to contribute to the program in a <br /> way that will ensure the confidentiality of the contributor. Two peop2 <br /> will be involved in the counting and receipting of the contributions . <br /> All contributions will be presented to the central agency office to be <br /> recorded accurately when received, and accounted for and reported on a <br /> regular basis . Contributions will be deposited and used to offset the <br /> costs of the program on a current basis toward the possible expansion} c <br /> Title III services . <br />
