MGERME::::DTUM.s0FeAGREEMENTgy <br /> This MEMORANDUM OF AGREEMENT Pollitzer, Chairperson, <br /> of the Human Services Advisory Commission Committee on community needs <br /> assessment, hereinafter called the Chairperson, Lisa Berolzheimer, herein- <br /> after called the Project Coordinator and Orange County, hereinafter called <br /> the County, for the purpose of conducting a Community Needs Assessment of <br /> Orange County Human Services. This assessment shall be carried out so as <br /> to accomplish the following. <br /> 1 . It must identify the service needs or problems existing <br /> within the county. <br /> 2. It must determine the scope and magnitude of these needs as <br /> evidenced by statistically relevant data, such as rate of <br /> service requests , victimization rates, health statistics, <br /> census information , Title XX Data, etc. <br /> 3. It must identify to whom these needs exist ; i .e. , age, race <br /> and sex. <br /> 4. It must provide an inventory of service providers (with <br /> information on each of their programs) as to their: <br /> a) services <br /> b) service area <br /> c) size and type of clientele <br /> d) size of staff (full-time, part-time and volunteers) <br /> e) hours of operation <br /> f) years in existance <br /> g) legal status (public, private) (profit or non-profit) <br /> h) location of Administrative Office(s) <br />
