P <br /> 5. CERTIFICATE OF FAMILY PARTICIPATION. The Certificate of Family Partici- <br /> pation shall expire at the end of 60 days unless within that time the <br /> Family submits a Request for Lease Approval. <br /> If a Certificate expires or is about to expire a Family may submit the <br /> Certificate to the PHA with a request for an extension. If the Family is <br /> making an effort to locate suitable housing and the PHA feels there is a <br /> reasonable possibility that the Family may find a suitable unit, an ex- <br /> tension of up to 60 days may be granted. A determination by the PHA <br /> that no further extensions shall be granted shall not preclude the <br /> Family from filing a new application for another Certificate. <br /> If an assisted family notifies the PHA that it wishes to obtain another <br /> Certificate of Family Participation for the purpose of finding another <br /> dwelling unit, or that it has found another unit, the PHA shall issue <br /> another Certificate or process a Request for Lease Approval, as the case <br /> may be, unless the PHA determines that the Owner is entitled to payments <br /> or funds on account of non-payment of rent or other amount owed under <br /> the Lease. <br /> [ Families determined ineligible by the PHA shall be notified by letter of <br /> the reason and that he or she has the right, within 10 days, to request <br /> an informal hearing. The PHA shall retain for three (3) years a copy of <br /> the application, notification letters, applicants response and, if any, <br /> the record of any informal hearing and a statement of final disposition. <br /> 6. LEASING OF A DWELLING UNIT. An applicant shall apply for a Certificate of <br /> Family Participation and when approved shall be responsible for finding a <br /> housing unit suitable to the holder's needs and desires in an area within <br /> the PHA's jurisdiction. A holder of a Certificate may select the dwelling <br /> unit he already occupies if the unit qualifies. <br /> When a Family has a unit it wants and the Owner has agreed to lease, the <br /> Family shall submit to the PHA a Request for Lease Approval signed by <br /> the Owner of the unit and the Family. A copy of the proposed lease, <br /> complete except for entry of the portion of monthly rent the Family shall <br /> be obligated to pay the owner, shall also be submitted to the PHA. <br /> When the applicant has accomplished the above, the PHA will accomplish <br /> the following: <br /> a. Determine whether the Contract Rent is approvable in accordance <br /> with the Fair Market Rent Schedule. <br /> b. Determine the allowance for any utilities or services which the <br /> Family is to pay directly. <br /> c. Determine that the unit is decent, safe and sanitary in accordance <br /> with the Housing Quality Standards. <br />
