We would very much like to <br /> participate again with the HSAC in active way as consultants. <br /> Since HSAC is still in its formative stages and is operating <br /> on still meager financial resources, we do not propose to bill <br /> HSAC for every consulting hour we spend rather with HSAC. <br /> Instead, we are/asking in this proposal for a fixed price of $1000. <br /> per consultant for a moo general set of services during the <br /> year 1981-1982. <br /> The consulting activities included )6(4/0% under this <br /> proposal are: <br /> 1. Regular consultation to and attendance at the HSAC <br /> steering committee and regular meetings (at least one consultant <br /> at each meeting and most times both consultants) <br /> 2. Preparation of post meeting evaluation forms and assistance <br /> to the steering committee in tabulating and discussing them. <br /> 3. Conduct at least one evaluation of HSAC by members and <br /> commissioners and the members. <br /> 4• Occasional consultation to task force and study committee <br /> chairpersons‘and their committees on as needed basis. <br /> 5. Other services mutually agreed upon by the consultants <br /> and the steering committee. <br /> This proposal does not include: <br /> Technical assistance or staffwork on particular HSAC projects <br /> such as block grants or needs assessment which may be Of0Xcfg( <br /> undertaken by the consultants for additional fees or orp. <br /> volunteer basis. <br /> We shall be happy to discuss this proposal with you and <br /> the steering committee. <br /> _ — <br />
