r 3 . the name, address, location, and telephone num- <br /> ber of the location to which the animal will be taken for the <br /> following five (5) days; <br /> 4 . the name , address, and telephone number of the <br /> collector; I,4 <br /> 5. the name , address, and telephone number of the <br /> person surrendering the animal. <br /> b. A written report containing the above information <br /> shall be delivered by the person collecting the animals to the <br /> Animal Shelter within twenty-four (24 ) hours of the surrender of <br /> the animal. <br /> c. A record shall be kept of the disposition of every <br /> animal which is collected. <br /> d. Every collector shall maintain a copy of all <br /> receipts and disposition records for one ( 1) year. <br /> B. A Quality Assurance program to ensure adherence to these <br /> standards shall be carried out within the Animal Control <br /> Division. <br /> C. The Animal Control Director shall be responsible for the <br /> full and proper application of this policy. Questions concerning <br /> the applicability or interpretation of this policy shall be the <br /> responsibility of the Animal Control Director. - <br /> XXI <br /> Section - Penalties .
