028 _ <br /> i -)`i RESPITE CARE <br /> SERVICE DEFINITION : A service which provides needed relief to care- <br /> givers of patients who cannot be left alone <br /> because of mental or physical problec :.and whose <br /> incomes preclude coverage under North Carolina's Medicaid <br /> eligibility standards. <br /> UNIT OF SERVICE: One ( 1 ) hour . <br /> SERVICE OBJECTIVES To provide relief time for unpaid caregivers who <br /> are caring for patients who require constant super- <br /> vision and who cannot be left alone either because <br /> of memory impairment or other problems that make <br /> them subject to wandering or make them dangerous to <br /> themselves or others , or because of physical <br /> immobility regardless of etiology , that renders them <br /> unsafe alone . These services are aimed toward <br /> preventing premature long-term institutionalization <br /> of the patient . <br /> PREFERRED TARGET <br /> GROUP; Caregivers of mentally or physically impaired adults <br /> (sixty years of age or older and/or their spouses ) <br /> at risk of institutionalization . <br /> SERVICE ACTIVITIES; Activities may include: <br /> 1 . Attendance and companion services for the patient <br /> in order to provide release time to the caregiver . <br /> 2 . Personal care services , including meal prepara- <br /> tion, to the patient of the caregiver . <br /> 3 . Patient assessment and care planning for the <br /> patient of the caregiver . <br /> 4 . Counseling and training in the caregiving role , <br /> including coping mechanisms and behavior <br /> modification techniques . <br /> 5 . Counseling in accessing available local , regional <br /> and state services . <br /> 6. Temporary placement of the patient in an adult <br /> day care , family care or domiciliary care facility . <br /> The above are limited to a maximum of forty-eight <br /> (48) hours of service per month per caretaker . <br /> Duration of the service period shall be unlimited <br /> for as long as the caretaker continues to qualify as <br /> a caretaker as defined by the service objective . <br />
