Orange County C.A.T. Driver <br /> Agreement <br /> This agreement entered into this 1st. day of October, 1987 by <br /> and between the County of Orange on behalf of the Department on Aging, <br /> hereinafter referred to as the"County" and Orange-Person-Chatham <br /> Mental Health Center, Inc. on behalf of Children 's Learning Center <br /> hereinafter referred to as "OPCMH. <br /> The purpose of this agreement is to outline the conditions under <br /> which the County and OPCMHC will cooperatively provide human service <br /> transportation under the C.A.T. (Coordinated Agency Transportation) <br /> system. <br /> SPECIFIC RESPONSIBILITIES OF ORANGE COUNTY <br /> 1 . To provide a certified driver for Children's Learning Center <br /> Southern Orange route Monday through Friday for daily pick-up and <br /> take home of clients between the hours of 7:15 - 9:00 a.m. and <br /> 2: 15 - 4:00 p.m. <br /> 2. To provide back-up drivers in case the regular driver is not <br /> available due to pre-arranged absences. <br /> 3. To employ a C.A.T. Transportation Supervisor to handle any <br /> problems and to assure there is a driver available. <br /> SPECIFIC RESPONSIBILITIES OF OPC MENTAL HEALTH CENTER <br /> 1. To reimburse Orange County at $6.50 per hour for a three hour <br /> minimum each day of operation within 30 days after billing. <br /> 2. To provide and pay for specialized training in the areas of <br /> client behavior interaction techniques with clients/families, med- <br /> ical management including CPR and first aid training and, any other <br /> procedures specific to the Children' s Learning Center program and <br /> route. <br /> - The term of this contract shall be from October 1 through June 30, <br /> 1988 unless sooner terminated by either party giving 30 days price-~ <br /> written notice to the other party of intention to terminate. <br /> IN WITNESS WHEREOF, Orange County and Orange-Person-Chatham <br /> Mental Health Center, Inc. have executed the Agreement of the <br /> date first written above. <br /> County of Orange Orange-Person-Chatham Mental <br /> Health Center <br /> 114117 BY: e ' G � <br /> Chair �- - �esi-noted By: � ' <br /> 7 Official Chair or D -signi/ed Official <br /> Date: .26'_ Y <br /> Date: <br />
