014 <br /> applicable in the performance of this contract. <br /> II. The University agrees that the School of Pharmacy will: <br /> 1) provide pharmacist staffing sufficient to deliver <br /> all of the activities identified in Appendix A: <br /> the staffing pattern to be followed will be agreed <br /> to by the Chairman, Division of Pharmacy Practice, <br /> and the Director of the Health Department; the <br /> activities of all School personnel within the <br /> Health Department shall be the responsibility of <br /> the School; assure that all personnel assigned by <br /> the School are adequately insured for liability/- <br /> malpractice; <br /> 2) assure that pharmacy services meet all applicable <br /> laws, regulations and standards; <br /> 3) insure the Chairman, Division of Pharmacy Practice, <br /> or his designate's availability on an emergency <br /> basis for administrative or pharmaceutical problems <br /> that arise which are directly related to the <br /> consultant pharmacy services at the Health Depart- <br /> ment; <br /> 4) provide the supervision and evaluation of under- <br /> graduate students assigned on a rotational basis to <br /> receive experience in the provision of consultant <br /> pharmacy services; student activities may include: <br /> assisting the pharmacist in providing consultant <br /> service, participate in projects to evaluate or <br /> improve therapeutics or pharmacy related activi- <br /> ties, present or participate in inservice education <br /> activities; <br /> 5) make available to the Health Department a roster of <br /> people (pharmacists and students) who will be <br /> working in fulfillment of this contract; all <br /> personnel changes in the roster will be submitted <br /> in advance to the Director of the Health Depart- <br /> ment; <br /> 6) render a bill monthly to the Health Department for <br /> the agreed upon fee. <br /> III. This contract shall be effective as of the 1st day of <br /> July 1987, and shall terminate on the 30th day of June <br /> 1988. It is agreed that either party may terminate this <br /> agreement by giving thirty (30) days notice to the other <br /> party of its intention to do so, said notice to be in <br /> writing. <br /> IV. This contract represents the entire understanding of the <br /> parties. Any modification of any type must be in <br /> writing and must be signed by both parties to be effec- <br /> tive. <br />
