089 <br /> 4.3 Format <br /> An EA is to be a concise document not to exceed 40 <br /> pages, containing the following: <br /> (1) a cover sheet including the name, address and <br /> telephone number of the person who can supply <br /> additional information. The cover sheet shall also <br /> contain: <br /> (a) designation of the statement as an EA; <br /> lb) title of the proposed activity that is the <br /> subject of the document; and <br /> (c) a list of any involved cooperating entities. <br /> (2) A section for each item listed in Section 4.2 . <br /> SECTION 5.
