026 <br /> Objectives: <br /> 1 . By March 30 , 1988, the Orange County Health <br /> Department 's Employee Wellness Program, HEALTH YOURSELF, will <br /> establish a collaborative relationship, with the Orange <br /> County Agricultural Extension Service, 4-H Program for the <br /> purpose of designing and planning the educational/incentive <br /> seat belt usage program for the target population. <br /> 2. By April 30 , 1988, the 4-H volunteers will have held <br /> a contest for its membership to design the Seat Belt <br /> Program 's logo and theme. <br /> 3. By May 30 , 1988, the Program Coordinator of HEALTH <br /> YOURSELF and the Adult and Youth volunteers of the 4-H will <br /> identify sponsors for the educational/incentive seat belt <br /> program among _Northern Orange County merchants. <br /> 4. By May 30, 1988, the educational campaign <br /> (consisting of payroll stuffers, grocery b <br /> announcements in county in-house newsletter, adverers, <br /> in local paper, posters, etc. ) will be developed. <br /> 5.5. By June 30, 1988, the HEALTH YOURSELF coordinator <br /> and the 4-H volunteers will be trained by UNC-HSRC to observe <br /> seat belt usage and will collect baseline data. <br /> 6 . By July 8, 1988, the initial educational campaign <br /> for the target population will be implemented. <br /> consist of payroll stuffers in County Employee 's (This <br /> July 8th1 <br /> paycheck, advertisement/news releases for July 6, 13, and 10,News of Orange newspaper, posters in Sponsor 's <br /> businesses and at the worksite, and ' <br /> 500 per supermarket pre week during the cahy aig stuffers at <br /> campaign. ) <br /> 7. On July 11 , 1988, the 4-H volunteers will begin <br /> observation of the target population seat belt usage. <br /> will be provided incentives/rewards/education. Rates wiJllrbe <br /> gathered and compared to baseline data. Observations will <br /> occur at different times and days. <br /> 8. By August 1 , 1988, the second educational campaign <br /> for the target population will be implemented b <br /> Department 's HEALTH YOURSELF Y the se <br /> payroll stuffers in County Employee's August 5, 1988 <br /> consisslth <br /> t of <br /> paycheck, advertisements and news releases in the August 3 <br /> and 10, 1988 News of Oran e. <br /> 9. By August 30, the target population will have been <br /> observed at least five times for seat belt usage , and users <br /> rewarded. Rates will be gathered and compared to baseline <br /> data. <br />
