.•4 - 036 ADDENDUM FN:OE88O322 TO ORANGE COUNTY C. A. T. VEHICLE LEASE AGREEMENT This is an addendum to the original contract between Orange Enterprises, Inc. (0. E. ) and the County of Orange. • The purpose of this addendum is to clarify criteria for eligibility of O. E. Riders, outline emergency procedures and clarify liability and collision insurance coverage. A. Criteria for eligibility of O. E. riders will include: 1. Availability of seats. 2. Inability of employee to provide own transportation 3. Where employee lives and his ability to get to pickup point. 4. Behavior on van. B. Following are agreed upon emergency procedures: Two-way radios will be installed in each van with the base located at the Department on Aging. In the event of a mechanical emergency, the van driver will radio to base for help. In case of a medical emergency, the van driver will first radio to 911 for aid and then to the base. Base operator will be responsible for contacting Orange Enterprises, Inc. At no time will the driver leave riders unattended. C. Insurance Orange Enterprises will provide liability insurance for their drivers. Orange Enterprises will provide a certificate of insurance showing the county as additional insured for vehicle liability and showing limits agreed upon by the County and Orange Enterprises_ The term of this addendum shall be. from Feb. 1, 1988, through June 30, 1988, unless sooner terminated by either party giving 30 days prior written notice to the other party of r� .-intention to -terminate_ - IN WITNESS WHEREOF, Orange County and Orange Enterprises, Inc., have executed the Agreement of the date first above - written County of Orange Orange Enterprises, Inc.
