• <br /> 2 NCAC 9L . 1005 (e) ; RESTRICTED AREAS; is proposed for amendment <br /> as follows : <br /> SUBCHAPTER 9L - PESTICIDE SECTION <br /> (e) No pesticide shall be deposited within al 1, 000 feet of <br /> any residence unless the aerial applicator has prior written <br /> consent of XIX/X14V5OXY54(Y/Of/ /91/7W/910/W/Og/X04X/s410 the head <br /> of household. The party who is contracting for the services of <br /> an aerial applicator shall obtain the written consent and forward <br /> it to the aerial applicator for record keeping purposes . The <br /> consent agreement shall include : <br /> ( 1) date of agreement!; <br /> (2 ) time period for which the consent is valid!; <br /> (3 ) location or designation of the residence!; <br /> 147// gY 0/95Y/ 4/s101 WFWX /X 0 /Y oW/Of// X910 / <br /> (4 ) name and EPA registration number of the pesticide (s) to , <br /> be applied as well as an explanation of possible <br /> hazards; <br /> ( 5) name, address , and telephone number of aerial <br /> applicator and/or contractor as well as contracting <br /> agent; <br /> ( 6) a statement that any consenting party may withdraw his <br /> or her consent at any time by notifying in writing the <br /> party which requested the consent. Upon such <br /> notification, the previous consent shall be <br /> invalidated; and r <br /> (7 ) signature of person consenting and person seeking <br /> consent.
