Upon the availability of a municipal or regional sewerage collection system, the <br /> subject wastewater treatment facilities shall be abandoned and all wastewater <br /> discharged into the municipal or regional sewerage system. <br /> 6. This permit shall become voidable in the event of failure of the soil to <br /> adequately absorb the wastes and may be rescinded unless the facilities are <br /> installed, maintained, and operated in a manner which will protect the assigned <br /> water quality standards of the surface waters and ground waters. <br /> 7. In event the facilities fail to perform satisfactorily, including the creation of <br /> nuisance conditions, the Permittee shall take such immediate corrective action as <br /> may be required by this Division, including the construction of additional or <br /> replacement wastewater treatment or disposal facilities. <br /> 8. The Raleigh Regional Office, phone no. 733-23]4 shall be notified at least <br /> twenty-four (24) hours in advance of backfilling of the installed subsurface <br /> disposal system so that an in-place inspection can he made of said system prior <br /> to backfilling. Such notification to the regional supervisor shall be made <br /> during the normal office hours from 8:00 a.m. until 5:00 p.m. on Monday through <br /> Friday, excluding state holidays. <br /> 9. The sludge generated form these treatment facilities must be disposed of in <br /> accordance with General Statute 143.215. 1 and in a manner approvable by the North <br /> Carolina Division of Environmental Management. <br /> 10. Diversion or bypassing of the untreated wastewater from the treatment facilities <br /> is prohibited. <br /> 11. The Permittee shall employ a certified wastewater treatment plant operator in <br /> responsible charge of the wastewater treatment facilities. Such operator must <br /> hold a certificate of the grade at least equivalent to the classification <br /> assigned to the wastewater treatment facilities by the Certification Commission. <br /> 12. Any monitoring deemed necessary by the Division of Environmental Management to <br /> insure surface and ground water protection will be established and an acceptable <br /> sampling reporting schedule shall be followed. <br /> 13. The issuance of this permit shall not relieve the permittee of the responsibility <br /> for damages to surface or groundwaters resulting from the operation of this <br /> facility. <br /> 14. The facilities shall be effectively maintained and operated as a non-discharge <br /> system to prevent the discharge of any wastewater resulting from the operation of <br /> this facility. <br /> 15. No traffic or equipment shall be allowed on the disposal area except while <br /> installation occurs or while normal maintenance is being performed. <br /> 16. Adequate inspection, maintenance, and cleaning shall be provided by the North <br /> State Utilities, Inc. to insure proper operation of the subject facilities. <br /> 17. An acceptable reserve area, equal in size to the approved disposal area, shall be <br /> maintained at all times. <br />
