NCD982093783-R2 <br /> STATE OF NORTH C • ' OLINA <br /> DIVISION OF WASTE • AGEMENT <br /> HAZA 1 I OUS WASTE MANAGEMENT PE ' IT <br /> Permittee: Owner: <br /> University of North Carolina at Chapel Hill University of North Carolina at Chapel Hill <br /> UNC Buildings 488 & 531, Estes Drive Extension CB 1000 300 South Building <br /> Chapel Hill, North Carolina 27514 Chapel Hill,North Carolina 27599-1000 <br /> Pursuant to the 15A NCAC 13A North Carolina Hazardous Waste Management Rules, an operating <br /> permit is issued to the University of North Carolina at Chapel Hill, owner and operator of a hazardous <br /> waste management facility located in the Cape Fear River Basin in Chapel Hill, Orange County on Estes <br /> Drive Extension, at latitude 35° 56' 03.71"N and longitude 79° 03' 28.21" W. <br /> The Permittee must comply with all terms and conditions of the permit. This permit consists of the <br /> conditions discussed in Parts I, II, III, IV, V, VI, VII, VIII; the applicable regulations contained in 15A <br /> NCAC 13A including the applicable provisions of 40 CFR Parts 260 through 264, 266, 268, 270 and <br /> 124; statutory requirements of N.C.G.S. 130A-Article 9 (Solid Waste Management Act as amended) and <br /> the attached Application. <br /> Applicable regulations are those which are in effect on the date of issuance of this permit [40 CFR <br /> 270.32(c) as adopted in 15A NCAC 13A.0113] and are attached. <br /> This permit is based on the assumption that the information submitted in the pelinit application and as <br /> modified by subsequent amendments (hereafter referred to as the Application) is accurate and that the <br /> facility will be operated as specified in the Application. Any inaccuracies found in this information <br /> could lead to the termination or modification of this permit and potential enforcement action [40 CFR <br /> 270.41, 270.42, and 270.43 as adopted in 15A NCAC 13A .0113]. The Permittee shall inform the North <br /> Carolina Depattnient of Environmental Quality of any deviation from or changes in the information in <br /> the application which would affect the Permittee's ability to comply with the applicable regulations or <br /> permit conditions. <br /> This permit is effective as of September 26, 2016, and shall remain in effect for ten(10)years until <br /> September 26, 2026, [40 CFR 270.50 as adopted in 15A NCAC 13A .0113] unless revoked and reissued, <br /> terminated or continued in accordance with 40 CFR 270.51 as adopted in 15A NCAC 13A .0113. <br /> C 1, Ail 11/ C I i 6 1 1 <br /> J ie S. looscief <br /> Date <br /> Hazardous Waste Section <br />
