a) name of the individual, agency; corporation or <br /> municipality making the request; <br /> b) approximate starting and completion date of the <br /> project and the approximate date when sewer or <br /> water utilities must be available; <br /> c) approximate cost of the water or sewer extension <br /> required; <br /> d) a map showing the general layout of the requested <br /> sewer or water extension from existing utilities <br /> to the project site; <br /> e) signed statement by the applicant that the requested <br /> __ water or sewer utilities will be adequate to serve <br /> the needs of the proposed project for sanitary <br /> purposes , fire protection and process water where <br /> necessary. Water or sewer utilities should be <br /> adequate to serve not only the initial needs of the <br /> project but also any anticipated expansion expected <br /> within five years; <br /> f) the type of project to be served by the utilities <br /> and the approximate number of employees when the <br /> project is completed and two years after completion <br /> of the project; <br /> g) the approximate number and average wage scale of <br /> persons employed in the categories of skilled labor, <br /> unskilled labor, clerical and management; <br /> h) the approximate number of Orange County residents to <br /> be employed; <br /> i) the cost of real property for the completed project - <br /> exclusive of land costs ; <br /> j) the cost of personal property for the completed project <br /> (equipment, machinery, facilities) ; <br /> k) a list of any hazardous or toxic chemicals to be stored <br /> or produced when the facility begins operations ; <br /> _ 1) gallons per day of anticipated water use when the <br /> facility is fully operational. <br /> Upon receiving a request for sewer or water utilities under the terms <br /> of this policy, the County Manager will gather the following addition <br /> information: <br /> a) a map and brief report identifying existing and anticipated <br /> areas of residential, commercial and industrial development <br /> that could be served by the requested sewer or water <br /> utilities ; <br /> b) approximate additional cost of installing oversized utilities <br /> to serve existing or anticipated development in the area of <br /> the proposed project; <br /> -2- <br />
