4 <br /> RESPITE CARE <br /> §ERVICE DEFINITION: A service which provides needed relief to caregivers of <br /> patients who cannot be left alone because of mental or <br /> physical problems and whose incomes preclude coverage <br /> under North Carolina's Medicaid eligibility standards. <br /> UNIT OF SERVICE: One (1) hour. <br /> ERICE OS,TEC7IVL: To provide relief time for unpaid caregivers who are <br /> caring for patients who require constant supervision and <br /> who cannot be left alone either because of memory <br /> impairment or other problems that make them subject to <br /> wandering or make them dangerous to themselves or others , <br /> or because of physical immobility regardless of etiology, <br /> that renders them unsafe alone. These services are aimed <br /> toward preventing premature long-term institutional- <br /> ization of the patient. <br /> ?REFERRED TAROT <br /> GROUP: Caregivers of mentally or physically impaired adults <br /> (sixty years of age or older or their spouses) ineligible <br /> for Medicaid according to North Carolina standards and at <br /> risk of institutionalization. <br /> SERVICE ACTIVITIES: A:tivities may include: <br /> 1 . Attendance and companion services for the patient in <br /> order to prcvide release time to the caregiver. <br /> 2. Personal care services, including meal preparation, <br /> to the patient of the caregiver. <br /> 3. Patient assessment and care planning for the patient <br /> of the caregiver. <br /> 4. Counseling and training in the caregiving role , <br /> including copying mechanisms and behavior <br /> modification techniques . <br /> 5. Counseling in accessing available local, regional <br /> and state. services. <br /> 6. Temporary placement in a family care or domiciliary <br /> care facility of the patient for no more than a <br /> total of thirty (30) days per year per patient. <br /> Items one (1) through five (5) above are limited to a <br /> maximum of twenty (20) hours of service per month per <br /> caretaker. Duration of the service period shall be <br /> unlimited for as long as the caretaker continues to <br /> qualify ws a caretaker as defined by the Service <br /> Objective . <br /> 8 <br />
