Each provider needs to apply their own <br /> unit cost into the formula . <br /> In order to keep the racammendld sliding fee scale as siuple as <br /> possible , we have not factored in formulas for: number cf <br /> dependents , medical expenses , loss of caregiver income, etc . <br /> Therefore, we are providing the sliding fee scale with the following <br /> implementation philosophy. The legislation requires that a minimum <br /> of 20% of the cost of the service (unit cost X number of ur :ts <br /> received in a given month) be charged to the caregiver . The income <br /> levels with their respective- percentages above the 20% level are <br /> 12cge.sted fees for service. The caregiver should be info:ued or <br /> the suggested fee.
