- c <br /> DSS Chore Supervisor y <br /> Primary Purpose <br /> • The primary purpose of this position is to implement and facilitate <br /> the Chore Service Program, which serves the aged/disabled population <br /> of our county. The worker acts ,as client' s social worker and provide. <br /> case management for Chore Service Cases , Coordinates the Chore Train- <br /> ing Program, Supervises all Chore Providers , and works in conjunction <br /> with the Long Term Care Screening (LTCS) Program Social Worker in im- <br /> plementing the LTCS Program. The worker acts as lead worker providing , <br /> training and consultation to the other social worker in this program <br /> area. Worker acts as back-up Protective Services Worker. <br /> II . Duties and Responsibilities <br /> A. Chore Service Program - 407 <br /> This program is designed to provide in-home care to the elderly or <br /> disabled clients who would not otherwise be able to remain in their <br /> home , or whose health or quality of life would deteriorate without <br /> this service . Funding for this program is through Block Grant and <br /> LTCS Program (Medicaid) . _ <br /> At present there are approximately 45 clients who receive services <br /> funded by Block Grant monies , and approximately 30 who receive Chore <br /> Services funded through Medicaid . Cases are assigned to worker based <br /> on the service being offered (Chore) . <br /> - i <br /> The worker is case manager and assures delivery of services for all <br /> Chore funded by Block Grant Funds . Responsiblitities in these cases <br /> involve: <br /> 1) Receiving and evaluating requests for services . Worker receives <br /> phone calls and office visits by individuals requesting services. <br /> Requests may also be received from other agencies such as Home Health <br /> Dept. on Aging , or from hospitals . • <br /> 2) Making a home visit to gather information about client concerning <br /> financial , environmental , physical needs , in order to determine appli <br /> cant' s eligibility for services . <br /> .3) Assessing client and family need based on information acquired fr <br /> client , family and other agencies . <br /> 4) Developing a Plan of Care/Service plan in order to assure client' <br /> needs are being met. Goals are clearly set, worker decides what <br /> specific services will be provided, by whom and how often. <br /> Implementing the plan of care by directing and controlling the way in <br /> which designated services are carried out. <br /> 5) Communicating with other professionals regarding plan of care in <br /> order to assure consistent services for the client. <br /> 6) Reassessing plan of care periodically to ensure appropriate modi- <br /> fications when necessary. Worker makes quarterly home visits and yea <br /> redeterminations of eligibility for services. Termination of services <br /> when no longer needed by client. <br /> 7 ) Referrals and follow-up calls to other agencies to assure provisi <br />
