Duties and Responsibilities in this area include : <br /> 1 ) Recruiting through newspaper adds , County Personnel office , <br /> Employment Security Commission and word of mouth. <br /> 2) Hiring qualified Chore Providers after interviewing, checking <br /> references , . <br /> 3) Ensuring the provider has initial Chore Orientation, verifying <br /> Provider' s previous training , or receives training in accordance with <br /> state requirements . <br /> 4) Providing monthly supervision and evaluation of job performance of <br /> untrained Providers and quarterly supervision of trained providers by <br /> home visits , reports from other professionals , and reports from the <br /> client. <br /> 5) Recommending promotion , new assignment , or termination of job <br /> based on provider' s performance . Worker schedules hours/days of serv- <br /> ice for each provider based on Plan of Care, worker' s capabilities . <br /> 6) Assuring training of providers in agency by developing course con- <br /> tent , contacting instructors , arranging for training facility, main- <br /> taining training logs atsd attendance records . Worker evaluates the <br /> training and makes appropriate efforts to improve skills and knowledge f <br /> of providers through course content. Worker arranges in-service train- i <br /> ing and advises workers of other training opportunities . <br /> 7) Maintaining Personnel Records of all Chore Providers , verifying <br /> necessary information such as references , training, work experience. <br /> 8) Checking all time sheets , and assist in completing payroll infor- <br /> mation. - <br /> Provision of this service is carried out by worker independently. <br /> Supervisor is available for consultation during individual advisory <br /> time and staff meetings . Guides used in the provision of the Chore <br /> Services Manual , Title XX Eligibility Regulations , Services Adminis- <br /> tration Manual , Chore Provider Handbook , SIS Manual , PDR. <br /> S . Coordination of Long Term Care Screening Program and Chore Services - <br /> Aged/Disabled clients in this area suffer from physical problems whit: <br /> would necessitate .placement in an Intermediate Care Facility should <br /> they not be able to receive care in their homes . <br /> Worker receives referrals from LTCS Program Social Worker. Home visit <br /> is made to assess need for Chore SErvices . Assessment based on infor- <br /> nation provided by LTCS social worker , client , family , other agencies , <br /> such as Home Health. <br /> Worker collaborates with LTCS social worker in order to assure con- <br /> tuinuity of services for clients . <br />
