PROPOSAL <br /> Part A - Project Description and Implementation <br /> 1. The applicant will provide 856 hours of Respite Care <br /> Service (units of service) to 17 Caretakers <br /> of older adults during January 1, 1987 - June 30, 1987 <br /> in ()ranee county(ies) . <br /> 2. The approximate number of hours to be provided to each <br /> Caretaker is: 50 <br /> 3. Please provide brief and succinct evidence that your <br /> agency has the capacity to provide the service. <br /> (i.e. , experience, trained staff, volunteers, etc. ) <br /> The Orange County Dept. of Social Services have been providing <br /> in-home care services for approximately 20 years, and for <br /> the past two years lead agency for the Long Term Screening <br /> Program. Staff has trained aides to provide this type of <br /> care and professional staff to handle the administration. <br /> 4. Explain how respite care services will be integrated <br /> with existing program operations. <br /> The Respite Care service would be administered under the <br /> Dept. of Social Services Long Term Care Screening and <br /> coordinated with the Dept. on Aging's Community Case Manage- <br /> ment service. The Respite Care Aides would be drawn from <br /> the DSS Chore Program and from the Home Health Agency's <br /> Private Aide Service. <br /> 5. Please list the specific steps your organizations will <br /> take to target and promote respite care services to <br /> unpaid caregivers caring for persons 60 years of age, <br /> or their spouse, not medicaid eligible. <br /> The Orange County In-Home Board,who has endorsed the need <br /> - for this service,has participating agencies - Health Dept. , <br /> Aging, Home Health Agency - with waiting lists of potential <br /> respite care clients. The DSS Chore waiting list would be <br /> reviewed for potential clients. The Dept. on Aging Newsletter <br /> and the DoA Case Management Service would publicize the service <br /> Local support groups would be contacted. <br /> 6. Please specifically identify how respite care workers <br /> will be recruited and trained. <br /> There would not be additional respite care workers recruited <br /> and trained for this grant. The existing trained DSS Chore <br /> Aides , Home Health and Home Assister Aides would be utilized <br /> for this service. If it becomes necessary, the DSS Chore <br /> Training tapes would be used with nursing instructors <br /> verifying the skills and competency. <br /> - 14 <br />
