NN <br /> No. 573 <br /> WM ID N/A <br /> SERVICE CONTRACT <br /> This contract, made and entered into this 6th day of October <br /> 19 86 by and between the Orange-Person-Chatham Mental Health Center and <br /> Oran a Count Personnel <br /> • <br /> That for the purpose and subject to the terms and conditions hereinafter <br /> set forth, the Orange-Person-Chatham Mental Health Center hereby contracts for <br /> the service of training for specialized foster parenting <br /> accept the terms of such a contract. and they <br /> First: The service to be performed by Orange County Trainer <br /> is as follows: <br /> Training, including 8 two-hour sessions plus preparation and coordination <br /> time, for prospective foster parents_ <br /> Second: This contract shall begin on the 6th day of October <br /> 19 86 , and unless sooner terminated by mutual consent or as hereinafter <br /> shall exist and continue until the 20th day of November 86 vided, <br /> provided that either party shall have the right to terminate this contract for <br /> service upon two weeks (14 days) notice in writing to the other <br /> party. <br /> expected that the contractee will conduct his/her duties in a manner consistent <br /> with the mission of the OPC Mental Health Center as a human service organization. <br /> Violations of the terms of this contract shall be grounds for immediate termination <br /> of the contract by the contractor. <br /> Third: The Orange-Person-Chatham Mental Health Center agrees to pay the sum <br /> of _ $20.00 per hr, <br /> serices og traifor a maximum of 52 hours or $1040.00 <br /> as full compensation for the- 7 <br /> upon receipt of an invoice which indicates the amount of monies owed to the con- <br /> tractee for services rendered. The invoice whould be sent by the 5th working day <br /> of each month after services are rendered to assure payment by the 15th of the <br /> month.
