Agreement <br /> This agreement entered into this 1st day of October, 1996. byo C d <br /> between the County of Orange , hereinafter re-erred to as <br /> "rnlinty:i and Orange--Chatham Comprehensive Health Services, Inc. , hereinaft: <br /> referred to as OCCHS <br /> The purpose of this agreement is to outline the conditions under <br /> which the County and OCCHS will cooperatively provide human <br /> service transportation to Uranae County' s elderly. infirmed. and <br /> disadvantaged under the C.A. T. (Coordinated Agency Transportation) <br /> system. <br /> SPECIFIC RESPONSIBILITIES OF the County <br /> 1. To lease the fallowing identified vehicles or similiar size <br /> at the hours 5oecified by each vehicle. <br /> 1984 Dodge Van (15 pass w/lift) - CAT #12 Lic. 91942-P <br /> VIN - 2B7KB33WXEK311123 <br /> SPECIFIC RESPONSIBILITIES OF OCCHS - <br /> 1. To provide all gas, maintenance, repairs and insurance <br /> (liability/collision - minimum of 90% of replacement cost) on <br /> leased vehicles. <br /> 2. To operate and schedule the use of the leased vehicle(-s) on <br /> behalf of human service clients and approved groups according - <br /> to the Orange County Transportation Board policy and procedures. <br /> 7. To allow C. A. T. program agencies access (timesharing/ridesharing) <br /> to leased vehicles) when not scheduled for use. at a rate <br /> estaolished by the Orange County Transportation 5dard and <br /> approved by the County Commissioners. <br /> 4. To complete all recuired vehicle oceration reports and sudmIt <br /> to the Cranoe County C. H. T. supervisor in the Department on acinc. <br /> The term of this contract shall be from October 1. 1926 through <br /> June .
