ATTACHMENT A <br /> TRANSPORTATION 018 <br /> 1 . Location of Service. Provision. This service shall be carried <br /> out for older persons in Orange County. <br /> 2. Service Standards. The delivery of this service shall be in <br /> accordance with Standards as adopted by the N.C. Division of Aging. <br /> 3. Compensation. The Contractor and COG expressly understand and <br /> agree that in no event will the total compensation and reimbursement <br /> paid hereunder by COG exceed the maximum of $23,299 Federal <br /> funds, $1 ,370. State funds for the specified service and shall <br /> constitute full and complete compensation for the Contractor's service <br /> hereunder. Local match of $2 ,741 Cash and -o- In- <br /> kind resources shall be required under this contract. <br /> 4. Reimbursement Rate Per Unit of Service. A unit cost reimbursement <br /> rate of -0- Dollars and fifty-seven Cents <br /> ( $0. 57 ) is agreed upon for every unit of service delivered <br /> to eligible recipients. COG will reimburse Contractor 90 % of this <br /> rate with Federal and State funds, an amount equalling $0. 51 <br /> per unit. <br /> 5. Scope of Service Units. It is agreed that Contractor will <br /> work towards delivering 48,000 trips of acceptable units of <br /> service to eligible recipients by June 30, 1987. <br /> 6. Changes to Reimbursement Rate or Units of Service. Any changes <br /> shall be as set forth in Section 10 of this Contract. Changes shall be <br /> subject to review by COG Advisory Council on Aging, COG Board and Contractor' <br /> Board. <br /> 7. Personnel . It is understood and agreed that Jerry M. Passmore <br /> shall represent the Contractor or Project Manager in the performance of <br /> this Agreement. Any change in such Project Manager shall be subject to <br /> agreement with COG for continuation of contract. <br />
