Collective Impact <br /> Collective Impact is a collaboration model that recognizes the need for formal structures for <br /> collaboration, because it does not happen without dedicated resources and shared goals and <br /> processes. The structure includes the six conditions outlined <br /> below: <br /> Common Agenda <br /> • Common Agenda •Keeps all parties moving towards the same goal <br /> • Common Progress Measures Common Progress Measures <br /> •Measures that gen m the TRUE outcome <br /> • Mutually Reinforcing Activities Mutually Reinforcing Activities <br /> • Communication •Each experbse is leveraged as part of the overall <br /> Communications <br /> • Backbone Support •This allows a culture of collaboration <br /> • Equity Backbone Organization <br /> •Takes on the role of managing collaboration <br /> Progress l In year two, FSA achieved numerous milestones for the project including: <br /> • Continued and new programming for summer 2016, including: <br /> o Year 2 of the Kindergarten Readiness Program <br /> o Year 1 of literacy-based summer programming and summer camp for Cohort 1 <br /> o A new partnership with the Department of Environment, Agriculture, Parks and <br /> Recreation to extend summer programming to a full day in Zone 4 <br /> • A significant partnership with the UNC Provost's Office to provide evaluation for the <br /> Family Success Alliance <br /> • Enrollment and support of 34 families in the Navigator program <br /> • Significant data and evaluation milestones, including: <br /> o Presentation of an overarching evaluation plan for FSA <br /> o Piloting of a shared intake system for Cohort 1 Navigator Program families <br /> o Initial implementation of Efforts to Outcomes, a cloud-based system for case <br /> management and data sharing between partners <br /> • Monthly newsletters and event information for partners and zone community members <br /> • An end-of-year zone celebration of Cohort 1 and their families <br /> Zone Priorities in Response to Gap Analysis Data <br /> Zone 4 Priorities Zone 6 Priorities <br /> 1.
