If the circumstances require immediate action services are implemented. Nursing services are usually covered by Medicare which gives the client and`-family support until the plan of care is approved. There is ongoing contact between the nurse, social worker and family. Pleans of care are revised frequently to meet the clients needs. A reassessment is completed every six months by the nurse and social worker to continue services. If the familykr client declines services this is documented and the family proceeds with their own plan. Our Advisory Committee consists of representatives from the following areas: Social Services Board (1 member), Senior Citizens Board (1 member), Health Department , () member), 2 members who have professional knowledge/ experience in gerontology, in-home care, or health issues related to aging or long-term care. These members are currently from a nursing home and a home health agency. Two members who are advocates for citizens who need or at risk of needing in-home care. These members are providing in-home care for their mothers.
