The assessment and plan along with a cover letter giving a <br /> summation of the case (written by the social.worker) along with the F1-2 is <br /> then sent to Raleigh for approval. If the circumstances require immediate , <br /> action services are implemented. Nursing services are usually covered by <br /> Medicare which gives the client and-family support until the plan of care is <br /> approved. <br /> There is ongoing contact between the nurse, social worker and family. <br /> Pleans of care are revised frequently to meet the clients needs. A reassessment <br /> is completed every six months by the nurse and social worker to continue <br /> services. <br /> If the family or client declines services this is documented and the family <br /> proceeds with their own plan. <br /> Our Advisory Committee consists of representatives from the following <br /> areas: Social Services Board (1 member). Senior Citizens Board (1 member), <br /> Health Department (1 member), 2 members who have professional knowledge/ . <br /> experience in gerontology, in-home care, or health issues related to aging or <br /> long-term care. These members are currently from a nursing home and a home <br /> health agency. Two members who are advocates for citizens who need or at risk <br /> of needing in--home care. These members are providing in home care for their <br /> mothers. <br /> The County Manager's designee and Directors of the Departments of Social <br /> Services, Aging and Health as ex,.cfiiciv members with voting privileges. <br /> The Long Term Care Social Worker is also a member but does not have voting <br /> privileges. <br /> Emphasis for 'Icontinuation grant period will be placed on increased public <br /> awareness, development of the private pay and optional populations.
