Implementation of Program <br /> The 'Social Worker, was hired February 25, 190. She made contacts with area <br /> agencies inviting, them to participate in. the Long Term Care Screening Program. <br /> Provider agreements were obtained with the following agencies: <br /> • Orange County Department of Social Services for Case management, screening, <br /> • chore, DME, Mobility aides, <br /> telephone alert. respite care- <br /> noninstiitutional <br /> • <br /> Home Health (Agency of Chapel Hill - Screening, DME, Aide, medical supplies, <br /> nursing OT, PT, ST <br /> Med Visit (Home Health Agency) - Screening, DME, Aide, medical supplies, <br /> • <br /> nursing OT, PT, ST . <br /> Medical Personnel Pool (Home Health Agency) - Screening, DME, Aide, medical <br /> • supplies, nursing OT,PT,ST <br /> • <br /> Agreements were procured with three home health agencies to give clients the <br /> freedom of choicelof providers. <br /> Professional Health Care Registry Respite Care - noninstitutional <br /> Meals on Whelels Meals <br /> Orange Congregations in Mission Meals <br /> Hillhaven Convalescent Center - Respite Care - institutional <br /> Other resources utilized are area churches, the Department of Aging (used for <br /> construction of mobility aides, minor home repair, Senior Center) hospice and <br /> telephone alert. Medical supplies and DME are purchased through area dealers <br /> and the least expensive company is utilized. Local stores are also used as a <br /> resource for DME lie. Hechinger has grab bars and hand held showers at a lower <br /> cost than the medical supply companies. <br /> Referral System <br /> The majority of our referrals have come from Home Health Agencies. Other <br /> referrals have been received from family members, friends, the Department of <br /> Aging, Orange Congregations in Mission, physicians, hospital social workers and <br /> Department of Sonial Services eligibility and social workers . <br /> Process for Screening/Case Management <br /> Referral is received by the Social Worker of Long Term Screening Team and a <br /> prescreening interview is conducted to explain the program and determine if the <br /> situation would be suitable for screening. If screening is desired an F1-2 is <br /> obtained, then a ,joint home visit is usually made by the Social Worker and Nurse <br /> and the assessment form is completed.- The social worker and nurse also develop <br /> the plan of care including input from the client and caregiver to best meet the <br /> clients' needs. The physician is contacted again to procure his approval of the <br />
