Two representatives <br /> serve as voting members (1 consumer and 1 provider) and two will serve <br /> as alternates (1 consumer and 1 provider). <br /> A portion of the council by-laws is attached for your reference which <br /> provides a definition of the terms provider and consumer. <br /> Please fill in the enclosed form and return by June 8th.
