c. In the case of any group or association of <br /> individuals, a list or roster of the members or affiliates of <br /> the organization, their individual addresses , the emergency <br /> medical training and experiences of each member, special <br /> medical skills or capabilities of each member , continuing <br /> medical education and training programs of the organization, <br /> communication capabilities and requirements of the <br /> organization's communication system, medical equipment and <br /> supplies available for use by the organization and all <br /> sources of income to the organization; <br /> d. Such other information as the Board , in its <br /> discretion, may require. <br /> Section 11 . Granting a Certificate. <br /> Upon receipt of an application for a certificate , <br /> the Board shall fix the time and place for hearing the <br /> applicant . No certificate shall issue unless the Board shall <br /> find, after the hearing, that the public convenience and <br /> necessity require that such certificate issue. Determining <br /> whether the public convenience and necessity require the <br /> issuance of such certificate, the Board shall consider and <br /> investigate the statements made in the application , the <br /> adequacy of existing emergency medical services, the <br /> experience and training of the applicants , the public need <br /> for such service and any other factors pertient to such a <br /> determination. If the Board finds that the public <br /> convenience and necessity require the issuance of such <br /> certificate, the Board shall grant a certificate upon the <br /> -9- <br />
