III. EVALUATION <br /> Family Planning <br /> Performance evaluation for the Orange County Health Department's Fan <br /> Planning Program, as documented by the performance evaluation reports of <br /> fiscal year 1979-1980 and fiscal year 1980-1981,continues to show a <br /> disappointing decrease in new participants in the program; decrease in tc <br /> persons receiving medical family planning services and total family planr <br /> medical visits. These indicators of course also affect the number of tc <br /> active patients. This latter number is therefore reported as less than t <br /> projected. However, the Health Department's target population for servic <br /> patients aged nineteen or less is on line with the expected performance a <br /> shows a definite increase in this age group's participation in the progra <br /> from the previous fiscal year. The patient continuation rate is 67%; the <br /> standard, 75%. <br /> During the first quarter of this current fiscal year, there have bee <br /> major disruptions in the clerical support provided to Orange County's Fain <br /> ° Planning Program. This is evidenced by the fact that the path flow from <br /> Orange County to the regional office has, on several occasions, been two, <br /> months behind. For one month of this fiscal year, in fact, no data at al <br /> was received until weeks after the expected submission dates. Our Staff': <br /> internal chart reviews have also determined that visit records are often <br /> in the chart and not forwarded at all to Raleigh. This of course results <br /> patient visits that are performed but never counted. Since the basis of i <br /> figures on the Performance Report are the visit records submitted to the <br /> State office of the Family Planning Program, it is clear that our first qt <br /> figures are under reported. <br /> Another major concern of staff has been the recent stringent enforcers <br /> of Orange County School District's policy not to allow students to leave <br /> school premises without parental permission. This has resulted in decreae <br /> clinic attendance from especially the Junior High and Middle Schools popu) <br /> Orange County initiated a major reorganization of family planning ste <br /> facilitate quelity assurance for the program and its activities; to attemT <br /> monitor and assure accurate, TIMELY data flow; and to initiate programmati <br /> changes with the aim of improved performance. <br /> Joyce Bulger, P. H. N. II has been given the responsibility for <br /> coordinating these activities. Through her leadership and with staff supp <br /> and participation the following actions have been taken: <br /> 1. Written policies and procedures for Family Planning Services have been <br /> reviewed and revised. <br /> 2. Two Family Planning Staff Meetings have been held to acquaint all staf <br /> with policies, procedures and expectations. <br /> • 3. Individual chart review is performed by clinical manager at the conclu: <br /> of each clinic session to assure adherence to protocols, accuracy and <br /> • timeliness of data flow. <br /> 4. Clinical indicator audits for documentation of rubella titer immunity, <br /> Family Planning counseling sessions, and follow-up of Class III Pap Sn <br /> performed by nursing supervisor as forwarded to regional office. <br /> , 5. Corrective actions by use of action plans initiated with staff to assur <br /> adequate follow-up of audit evaluations. <br />
