By June 30, 1982, there will be 1 ,879 active patients enrolled in Oran: <br /> County Health Department Family Planning Program. <br /> 6. By June 30, 1982 provide 4,000 medical service clinic visits to active <br /> patients enrolled in Orange County Health Departments Family Planning <br /> 7. During fiscal year 1981-1982, 90% or 558 family planning patients aged <br /> nineteen years or less will have rubella immunity assessed and/or rece <br /> counseling on the importance of establishing immunity. <br /> Methods: a. Individual chart audit by clinic manager at end of each <br /> clinic session. <br /> b. Quarterly audit by personal health staff quality assurrar <br /> committee. <br /> c. Biannual audit by OHS <br /> 8. During fiscal year 1981-1982, 90% or 558 of the patients aged 19 years <br /> less receiving medical family planning services will have a documented <br /> counseling session prior to or at the time of receiving any family plan <br /> method. <br /> Methods: Same as Methods, Objective 7. <br /> 9. During fiscal year 1981-1982, 100% of the reported Class III, IV, or V <br /> Pap Smears will have documented follow-up and further diagnostic study <br /> and/or treatment in accordance with family planning policies. <br /> Methods: Same as Methods , Objective 7. <br /> b.
