ry <br /> IMPLEMENTATION PLAN <br /> HOMEMAKER-HOME HEALTH AIDE <br /> Contractor: Orange County Dept. of Aging Area Covered: Orange County <br /> 300 West Tryon Street <br /> Hillsborough, NC 27278 <br /> 919/732-9361 <br /> Period: 10/1/80 - 9/30/81 <br /> Method of Delivery: <br /> Orange County Department of Aging will use Title Ill funding to support <br /> a half-time Homemaker Coordinator who will be responsible for the in- <br /> service education of homemakers, coordination for appropriate placement <br /> of participants with homemakers, and the counseling of participants <br /> needing home care. The homemakers will come from a corps of personnel <br /> trained to render supportive aide to older persons who have lost some <br /> ability necessary to maintain independent living. These homemakers will <br /> be under the supervision of a physician, registered nurse or family <br /> member. The program will supplement the professional nursing and physics <br /> therapy services provided by the Home Health Agency and the County <br /> Health Department. <br /> In cooperation with the County's health agency, a brochure entitled <br /> In Home Services will be updated and distributed. Joint referrals will <br /> exist between the Department of Aging, the Department of Social Services, <br /> the Home Health Agency, Orange-Chatham Comprehensive Health Services, <br /> and the County Health Department. <br /> Project Income: <br /> Service participants will be allowed to contribute to the program in a <br /> way that will ensure the confidentiality of the contributor. Two people <br /> will be involved in the counting and receipting of the contributions. <br /> All contributions will be presented to the central agency office to be <br /> recorded accurately when received, and accounted for and reported on a <br /> regular basis. Contributions will be deposited and used to offset the <br /> costs of the program on a current basis. <br />
