' y <br /> '1 1 <br /> ATTACHMENT A-1 : Special Conditions - Including Requirements for Prog <br /> Reporting to COG <br /> As part of the Contractor's responsibility to perform the scope of WOT <br /> outlined in Attachment A-2, the Contractor agrees to meet the followir <br /> special conditions: <br /> 1 . To abide by the legal provisions and regulations of Title Ill of <br /> the Older Americans Act of 1966, As Amended; <br /> 2. To design the delivery of services outlined in Attachment A-2 in <br /> a manner that will encourage the participation of minority and lo <br /> income older persons; <br /> 3. To furnish the COG with monthly statistics for each service on th <br /> numbers of unduplicated persons served (including a breakdown of <br /> ority and low-income older persons), the volume of service given <br /> other required or essential information; <br /> 4. To maintain a distinct financial record for Title Ill and to furn <br /> COG monthly with a financial report; <br /> 5. To furnish COG with a quarterly narrative on program activities; <br /> 6. To furnish COG with quarterly summaries of working relationships s <br /> other community service providers for older persons and resources <br /> pooled to complement Title 111 funding; <br /> 7. To furnish COG with minutes of community interagency meetings; <br /> 8. To participate in contract monitoring sessions with the COG to be <br /> conducted quarterly for new services and semi-annually for contin- <br /> uation services, and to accept occasional on-site visits by COG st <br /> 9. To attend monthly meetings of Title III contractors. <br />
