35 <br />CHAPTER 5 <br />CARE MANAGEMENT <br />Mission <br />The mission of Care Management is to provide access to services 24/7/365; review <br />service requests and make decisions regarding medical necessity. to promote the use of <br />services which meet standards of quality and best practice. ~'~ -` <br />,' <br />;~ <br />Purchaser Standards ~ '` <br />~! <br />OPC Area Program is currently in compliance with all applicable local state and federal <br />statutes and rules, or will dedicate resources to the correction of any known or identified <br />deficiencies. ~`~ ~~~~>..,~ <br />Current Operations ~ ~' <br />Care Management includes the service management functions of Screening, Triage, <br />Access and Referral, Authorizations and.'Care Coordination. <br />Screening, Triage, Access and Referral `,, ' <br />,~ <br />Screening, Triage, Access and Referral (STR) is available to consumers 24/7/365 via <br />toll free telephone lines which allows prompt~_and easy access to needed services and <br />supports from a provider of their choice within~,a130-minute/30 mile commute from their <br />home. In accordance with Enhanced Services ,Implementation Update #14 providers of <br />enhanced services may perform screening, triage and referrals. Telephone calls are <br />answered y with'in` -„ five rings or 30-seconds. Additionally, Tele <br />Typewriter/Telecommuriications Device for the Deaf (l-fY/TDD) or language <br />interpretation services,. are available in accordance with Title VI guidelines. STR <br />administrative staff receive 'consumer registrations from providers of enhanced services <br />and enter this°information`info OPC's information system. <br />`\ <br />STR/Authorizations,,:for Developmental Disabilities Community <br />~ ,~ <br />. _ - <br />OPC employs five.:;..; staff to perform STR and Authorizations functions One <br />STAR/Authorizer h`as expertise in developmental disabilities and is a Qualified <br />Developmental Disabilities Professional. This person authorizes services for individuals <br />with developmental disabilities including developmental day and developmental therapy. <br />Most screenings for individuals with DD are conducted face to face; however, there are <br />instances where it is necessary to conduct telephonic screenings. Additionally this <br />person maintains the CAP prioritization tool, updates it, notifies families when selected, <br />and explains the process for getting a case manager. This staff person receives MR2s <br />(an assessment tool used to determine whether an ICF/MR ((Intermediate Care Facility <br />35 <br />
