042 <br /> (3) A medical certificate signed by a physician, licensed to <br /> practice in this state, within seven (7) days of the date of <br /> the application. The certificate shall state that the <br /> applicant was examined by the certifying physician and that <br /> the applicant is free from reportable communicable diseases <br /> that are specified on North Carolina Communicable Disease <br /> Report Card (DHS form 2124 - Revised 1-84) , which cards are <br /> available from the Orange County Health Department. <br /> The information required by this subsection shall be <br /> provided at the applicant's expense. <br /> (D) The Clerk to the Board of County Commissioners shall transmit <br /> a copy of the application to the Sheriff for an investigative <br /> report. The Sheriff ' s Department shall, within a reasonable <br /> time, not to exceed forty-five (45) days, report the results of <br /> its investigation to the Clerk to the Board of County <br /> Commissioners. <br /> (E) An application in proper form shall be submitted to the <br /> County Manager together with all reports required by this <br /> section.
