The bidder shall acknowledge receipt of this addendum inthe space provided on the <br /> Request for Pricing. Failureto do so may disqualify the bidder. <br /> Bid Due date has been revised to: Return pricing to Angel Barnes no later than NOON on Friday, <br /> July 8. 2016. <br /> Schedule: <br /> The construction schedule is as follows: <br /> Start Date: July 15, 2016 <br /> Completion Date: October 1, 2016 <br /> General Items: <br /> 1. General contractor shall include weekly meetings with CPM (Capital Project Manager) and <br /> customer for weekly updates. <br /> 2. General contractor shall provide pricing to include a complete phased schedule, with no less than <br /> two weeks look ahead for planning and scheduling with customers, signs, IT, and furniture moves <br /> as necessary. <br /> 3. General contractor shall NOT include any pricing for mural in bid. <br /> 4. General contractor shall be responsible for providing materials to protect all areas, include <br /> furniture, equipment, or other items within the space from paint. <br /> 5. General contractor shall paint the brick walls if they have existing paint, if the walls are brick and <br /> are not painted contractor shall NOT paint these walls. <br /> 6. General contractor shall schedule the dental clinic to be completed first. This work must be <br /> completed by August 4, 2016. This is the dental clinic only, all other areas to be completed by <br /> October 1, 2016. <br /> Specification Items: <br /> 1. General contractor shall including painting the chair rail to match the wall color. <br /> 2. General contractor shall include adding handrails to the (5) stairwells on the existing walls without <br /> handrails. Handrails must have the rail to wall included, and must be painted to match existing <br /> handrails. <br /> 3. General contractor shall paint the restroom partitions that are currently painted. If the partitions <br /> are not currently painted they are to stay unpainted. <br /> 4. General contractor shall paint all window trims to match the walls, with the existing sheen (semi- <br /> gloss) as trim has today. This is for the window trims only; all other trim shall be painted per the <br /> base specifications. <br /> 5. Contractor shall paint the walls with an eggshell finish. <br /> 6. General contractor shall include replacing all existing wall cove base with Johnsonite (John CB- <br /> TB1) or equivalent to. All cove bases shall be 6" width. The color shall be peppercorn or <br /> equivalent. <br />
