Hillsborough,North Carolina 27278 Attn: Marabeth Carr,ASLA—Landscape Architect Reference: Proposal for Construction Materials Testing (CMT) Services Fairview Park Parking Area Hillsborough,North Carolina Summit Proposal No.: T16-0065 Dear Ms. Carr: Summit Design and Engineering Services, PLLC is pleased to present this proposal to perform Construction Material Testing (CMT) services for the above reference project. As a provider of professional engineering services, Summit brings a proven successful CMT team led by experienced engineers, engineering technicians and project managers. The purpose of our services is to provide construction observation and testing services, as required by code, to assess compliance with project documents. Testing will be performed as a means of quality assurance testing on behalf of the owner. The following information forms the basis of this proposal and our scope of services: • Review of the plans and specifications for the project dated March 8, 2016 and May 16, 2016 respectively; • Summit currently provides geotechnical engineering services for the project; therefore, we have an understanding of the existing subsurface conditions; • Past experience on projects of similar size and scope. PROJECT DESCRIPTION Based upon our review of the provided site plan, we understand the project will consist of a 59 space, surface parking area and access road from the existing Orange County Public Works access road. Final grade elevations were not provided; however we assume+1- 5 ft. from existing grade. Clearing for boring access will be necessary from both the Fairview Park side and the Orange County Public Works access road side. We will attempt to get as close as possible to proposed boring locations, as well as, minimize any visual damage to site as a result of drilling operations. SCOPE OF SERVICES Our scope of services is described below and provides the basis of our fee estimate. It should be noted our services will exclude: (1) supervision, direction, or acceptance of the contractor's work;
