Goal: Zone Navigators will help families build protective factors and increase resiliency <br /> in their lives and communities. <br /> i. Provide training on the framework of Resiliency(Risk vs Protective Factors). <br /> ii. Provide tools for discovering existing strengths and protective factors with <br /> families. <br /> iii. Identify protective factors needed to support individual and family resilience, <br /> and strategies for increasing protective factors. <br /> 7. Provisional status and monitoring of agreement <br /> a. Hiring: Provider will coordinate with OCHD to recruit, interview and select appropriate <br /> candidates. <br /> b. Probationary period: Provider will follow internally established protocols for employee <br /> probationary period. <br /> c. Disciplinary action: Provider will follow internally established protocols for monitoring <br /> any activity or behavior that requires disciplinary action and report that to OCHD. <br /> d. Termination: Provider will follow internally established protocols for termination and <br /> will notify OCHD. Any final termination decision will be made after a meeting with OCHD <br /> program staff. <br /> e. Position Open: Should the position become open due to termination or attrition, <br /> provider will re-open the position and re-start the hiring process outlined above with an <br /> expectation of filling the position in a 30-day time period. Assuming such protocols are <br /> followed there would be no interruption of overhead payments during a period when <br /> the position was open. <br /> f. Navigator reporting: Navigator will provide written feedback to OCHD regarding any <br /> perceived violation of the agreement and OCHD will provide guidance to develop <br /> solutions amenable to all parties. <br /> OCHD Project Coordinator will: <br /> 1. Provide assistance with logistics for monitoring navigator activities and performance, including <br /> identification of priority programs and recruitment/referrals and information for Navigators to <br /> share with families. <br /> 2. Compile program, service, and other opportunities for Zone families from community partners <br /> and send to Provider to share with Navigators. <br /> 3. Provide supplies for navigators as needed for community events and outreach. <br /> 4. Identify ongoing learning and training opportunities for zone navigators and send to Provider. <br /> Personnel: 2 part-time zone navigators <br /> Cost: $57,893.00 to be paid in four, quarterly installments of$14,473.25, beginning on or about July 1, <br /> 2016, and after receipt of quarterly expense and performance reports thereafter. <br /> Budget Categories <br />
