DocuSign Envelope ID:7F09D547-D59D-44A9-AA51-DAABA2B54B42 [Departmental Use Only] TITLE TRU Club Advisor FY 2016-17 ORANGE COUNTY CONTRACT UNDER$1,000.00 NORTH CAROLINA THIS AGREEMENT, made and entered into this first day of July, 2016, ("Effective Date")by and between Orange County, North Carolina, a body politic and corporate organized under the laws of the State of North Carolina, (the "County"), party of the first part; and Linda Karcher (the "Provider"), party of the second part; WITNESSETH: For the purpose and subject to the terms and conditions hereinafter set forth, the County hereby contracts for the services of the Provider, and the Provider agrees to provide the following services to the County in accordance with the terms of this Agreement, time being of the essence: The services and/or materials and/or construction (hereinafter referred to collectively as "Services") to be furnished under this Agreement are as follows: 1. The Provider will serve as the advisor for the Carrboro High School Tobacco Reality Unfiltered (TRU) Club during the 2016-17 school year. 2. The Provider, with support from Orange County Health Department (OCHD) staff, will facilitate the completion of at least two youth-led tobacco prevention activities or projects within the school and at least one in the community. 3. The Provider will participate in a TRU Club Advisor training in fall 2016 for guidance on conducting TRU activities and will communicate regularly throughout the school year about plans and accomplishments. 4. The Provider will hold TRU Club meetings at least once per month, securing space at the high school for meetings, establishing a regular meeting time with school administrators, and advertising meetings. 5. The Provider will be responsible for recruiting students into the TRU Club. 6. The Provider will ensure that all TRU Club members submit signed consent and transportation forms prior to participating in events hosted by OCHD and notify OCHD staff immediately of any known disciplinary issues that violate the signed agreement. 7. The Provider will require that all TRU Club members participate in at least one Tobacco 101 training offered by OCHD. 8. The Provider will refrain from using tobacco products during TRU activities. 9. OCHD will provide snacks, incentives, and supplies for activities upon the request of the TRU Club Advisor as funds permit. The term of this agreement rendered shall be from August 17, 2016 to June 30, 2017. Provider represents and agrees that Provider is qualified to perform and fully capable of performing and providing the services required or necessary under this Agreement in a fully competent, professional and timely manner to the satisfaction of the County.
